Specialist Liaison Nurse (SLN) We are recruiting a brand-new role for a nurse with previous experience of delivering patient care in Ophthalmology.
The main scope of this role is to be responsible for delivering a Patient Programme, to support patients with Wet AMD as well as their carers, ensuring a platform for the continuity of care is established at the very beginning of the patient journey.
The Specialist Liaison Nurse will ensure their patients are confident in managing their disease and treatment outcomes.
Activities will include: Initially this role will be predominately based from home, employing a range of omnichannel resources to support, educate and be the immediate point of contact for Wet AMD patients and their carers.
Deliver ongoing and tailored contact and touch points to those patients and carers during the entire treatment journey, including pre -treatment coordination and reminders of any clinical tests.
Working closely with MDTs- reporting any side effects or issues whilst offering advice to patients and carers in relation to the early identification of adverse effects.
Provide education and tools to enable the patient to get the most from their therapy- this could include education on lifestyle strategies, ensuring the importance of compliance and regular assessments as well as checking the need for any enhanced psychological or emotional support.
Ensuring all reporting is recorded and the Patient Support Programme Patient Portal is successfully deployed to ensure ongoing connectivity to the patient and their carers.
Key requirements: Current Registered General Nurse Qualification, minimum of a Band 6 level.
Recent experience and specialist knowledge of Ophthalmology.
(Wet AMD experience is preferred.) Previous experience and evidence of success in a nursing function within the Pharmaceutical Healthcare industry and extensive understanding of the NHS and its objectives and targets.
Previous experience of delivering a Patient Support Programme is desirable.
Highly IT literate, ability to engage with patients virtually through various omni channels.
High levels of flexibility.
Excellent interpersonal and communication skills.
Although this role will be predominately based from home, a current UK driver's licence with no more than 6 points and an ability and willingness to travel to Trusts and patients' homes where appropriate will be needed.
